4Accomplishmentsfor2013Thislistisnotexhaustivebutitgivessomeideaoftheimpactofouractivities.SeniorCommunityService181seniorsprovidedover112822hoursofone-to-oneservicevaluedatnearly1.5milliontoover300at-riskchildreninSuffolkCountyat48communitysitesresultinginimprovedtestscoresfor95ofthechildren.Seniorsprovided29236hoursofassistancevaluedatover380000to81eldersinSuffolkCountytohelpthemremainintheirhomes.Morethan30familiesreceived3800hoursofservicevaluedatover50000toassistthemincaringforafrailelderlyfamilymemberavoidinghigh-costinstitutionalplacement.BasedontheSuffolkCountyLivingWageof13.12in2013.HousingInNewYorkCityandonLongIsland315peopleinrecoveryfrommentalillnessresidedinsafeaffordablehousingwithflexiblesupportservices.Afterlearningneededskills50oftheresidentsofourtransitionalhousingprogramsobtainedpermanenthousingwithinayear.OutreachandAdvocacyPeeradvocatesinSuffolkreachedouttoover1000peoplewithmentalillnessprovidingthesupportnecessarytoremainindependentinthecommunity.HungerandHomelessServicesOver900peoplewhowerehomelessoratriskofhomelessnessreceivedhousingemploymentandpeerservicesthroughourhomelessoutreachprograms.OurfoodpantryandsoupkitcheninSuffolkCountyprovided9576mealstohungryindividualsandfamilies.PersonalRecovery-OrientedServicesPROSOver400peopleinrecoveryparticipatedinintegratedrehabilitationsupportandtreatmentserviceswithover70peoplesecuringsafeaffordablehousinghighereducationandemployment.Memberswereempoweredtoachievetheirgoalsandreportimprovedrelationshipswithfamilyandfriendsandincreaseduseofcommunityresourcesandevents.Therewasadramaticreductionintheuseofpsychiatrichospitalization.FinancialServicesInSuffolkCounty350peoplewereabletoachievefinancialstabilityandtoregaincontrolovertheirfinancesavoidingpsychiatrichospitalizationandhomelessness.CareCoordinationOnLongIslandandinNewYorkCityover2000peopleweresupportedintheirrecoveryfrommentalillnessandlinkedtoservicesandcommunityresources.EmploymentOf85peoplereceivingsupportedemploymentservicesapproximately48obtainedemployment.ClinicalServicesOver500peoplereceivedclinicaltreatmentinNewYorkCityandSuffolkCountysupportingtheminachievingtheirgoalsandremainingincommunitysettingsoftheirchoice.MentalHealthServicesPeerServicesPeersareessentialparticipantsinthedeliveryofallourmentalhealthservices.PeerCounselorsprovideone-to-onesupportandarerolemodelsforindependentlivingteachingtheskillsrequiredtopromotewellnessandfosterhopeforabetterlife.Theyrelateonapersonallevelsharingtheirownlifeexperience.PeerAdvocatesteachself-helplinkpeopletoresourcesandfosterself-advocacy.Todaymorethan35percentofourstaffmemberscouldbeconsideredpeers.Manyareself-identifiedbutothersusetheirexperiencewiththementalhealthsystemtoinformtheirpracticeanddonotchoosetolabelthemselves.OutreachandAdvocacyInSuffolkCountythroughourPOWERPeerOutreachwithEveningRecreationProgramFederationprovidesemergencyassistanceadvocacyreferraltohousingandinformationtohomelessindividualsandfamiliesaswellaspeoplewithmentalillness.Aweeklymealprogramvocationaleducationalandemploymentservicesareavailableaswellasafoodpantry.CareCoordinationTheSuffolkandNewYorkCityCaseManagementProgramsassistindividualsrecoveringfrommental
